In 2021, a surge in suicide attempts among children and teens was a call to action for Cook Children’s Health Care System.

In March of that year, a record 43 patients were admitted to Cook Children’s Medical Center – Fort Worth after attempting suicide. This staggering figure was nearly double the number of suicidal patients seen previously. Even worse, kids were dying.

In 2020, Cook Children’s recorded seven suicide deaths, marking the first time suicide was the leading cause of trauma deaths at the hospital, surpassing child abuse and car wrecks.

“We started the journey back in 2021, when we were seeing the devastating increases in kids and teens trying to take their own lives,” said Wini King, co-host, executive producer, and Chief of Communications and Employee Connections at Cook Children’s.

The Joy Campaign was launched as a suicide prevention communication initiative. Joy, an acronym, reminds us all to ‘Just Breathe,’ ‘Open Up,’ and ‘You Matter.’

After a year of articles, media interviews, and speaker requests from the public, the Joy Campaign grew into the Raising Joy podcast. Feb.11 marks the 100th episode.

Hosted by King and Kristen Pyrc, M.D., medical director of outpatient psychiatry at Cook Children’s. The podcast tackles tough topics like depression, anxiety, stress, bullying, and social media.

Dr. Pyrc recalls treating patients as young as 6 and 7 for suicide attempts.

Anecdotal stories like this one are backed up by data collected through Cook Children’s Emergency Department (ED). In 2022, more than 30,000 patients in the ED, aged 8 and up, were screened using the Ask Suicide Questions (ASQ) tool. The results were alarming. Six hundred and forty kids screened positive for suicide risk and a crisis contact information sheet was provided to more than 2,000 families.

Dr. Pyrc and King recall seeing the shocking data.

“It was right here in our very backyard, in front of our very eyes,” said King. “It was a lot.”

Hard conversations

Three years later, the Raising Joy podcast has listeners in 60 countries and more than 23,000 downloads.

King and Dr. Pyrc interview medical professionals and families who share their personal experiences. They both say those personal stories have been the most impactful.

“We have had a lot of people who lost babies, who are taking care of catastrophically ill babies, but still manage to smile, encourage and inspire,” King explained.

Dr. Pyrc adds some of her favorite episodes have been speaking to teens, and hearing their insights.

While the goal is to make a difference in the community, Dr. Pyrc shared that these conversations have also impacted her parenting.

One of her most memorable guests was Brad Hunstable, a Fort Worth father who lost his son Hayden to suicide just four days shy of his 13^th birthday. Hunstable was one of the first people to open up and share his story for the Joy Campaign. He later appeared as a guest on Raising Joy.

“That was very impactful for me, and it changed more how I parent than how I practice being a psychiatrist,” explained Dr. Pyrc.

She shared how this episode impacted her conversations with her kids, especially when they are sad or seem down. Dr. Pyrc emphasized the importance of open communication and nonjudgmental responses to children’s questions.

“I do not want a child ever to think that there is anything that they can do or say, that would justify them having to think, ‘I need to because my mom or dad are disappointed,’” King said.

While the conversation around mental health has increased since 2021, the need for resources continues to rise.

In 2023, Cook Children’s screened more than 32,000 patients in the ED with the ASQ tool. Of those, 713 screened positive for suicide risk and a crisis contact information sheet was provided for 2,221 kids struggling with sadness and worry. Twenty-four children were suicidal but were visiting the ED for a different medical complaint.

While 2024 data is still being compiled, Dr. Pyrc and King continue to focus on informing and educating the public about mental health. King hopes that parents can relate and find solace in their own experiences by hearing from others.

“Hearing the incredible life experiences of others helps you understand your circumstances are not impossible,” King said.

What kind of impact can a single pair of shoes have on a child’s life? According to Sam Sayed, a medical student at the TCU Burnett School of Medicine, the impact can be life-altering. At least this was the case for his own life. 

On this special episode of Raising Joy, Sam talks to us about his nonprofit, Dayna's Footprints, which he started with his brother Sharif Sayed. Their mission is, “To combat learning inequity by providing students shoes to wear proudly, subsequently increasing students’ self-esteem and enthusiasm for attending school.”

In this episode of Raising Joy, we discuss a new peer-to-peer support program that is underway in Cook Children's Pediatric Intensive Care Unit (PICU). Chaplain Amanda Payne Lindsay and Katelyn Terry, RN, join us to talk about the program's goals, how it works, and the impact it's having on PICU staff.

The PICU can be a traumatic environment for health care workers, and this program is aimed at limiting the impact of stress, depression, anxiety, and burnout. The program trains appointed PICU employees to serve as supportive peers to lean on in times of need.

Navigating next steps as a family after inpatient psychiatric care can be tricky. In this episode of Raising Joy, we talk to Helen Ramsbottom, RN, and Melody Hackfeld, RN, both case managers with Cook Children's psychiatric department, about how to manage a child's mental health care after they are released from an inpatient psychiatric unit. They also discuss resources and support groups for parents, such as Links of Hope.

In addition, we learn about the ASQ tool, which is used by nurses in the emergency department at Cook Children's to identify suicidal patients. The tool is comprised of questions that are asked to every patient, regardless of their reason for visiting the emergency department. Since 2017, 640 suicidal children have been identified and received the support they needed. Managing emotions as a parent when your child receives a complex medical diagnosis can be incredibly challenging. Grant Halliburton Foundation was established in 2006 in memory of Grant Halliburton, a Dallas teen who battled depression and bipolar disorder for several years before his suicide death at the age of 19. The Foundation that bears his name works to help families and young people recognize the signs of mental illness through a variety of avenues including mental health education, collaboration, encouragement, and information.

When Melarie Odelusi’s daughter was diagnosed with aplastic anemia in 2020, she didn’t know what the future held in store for her family. One thing she did know – it was going to be critical for her to take care of herself. While doctors focused on her child, Melarie and her husband made every effort to ensure they were mentally strong through the storm. Today, Melarie is a member of Cook Children’s Family Advisory Council and uses her experience and perspective to help improve the hospital for all patients and families.

As important as a child’s mental health is, it’s equally as important to look at how their journey impacts the people around them -- their caregivers. The ACEs Task Force of Tarrant County developed a video series called The Roadmap for Children’s Mental Health to help families navigate this health issue. These are some tips on how to take care of yourself to ensure you can be the best support system as a caregiver.

“Parents often have a lot of guilt, they have a lot of worries, they have a lot of uncertainty that they’re going through,” said Steve Chennankara, D.O., child psychiatrist at Cook Children's Psychology Clinic - Fort Worth.

First, recognize stress and model appropriate coping skills. Parents need to take care of their own mental health because as a caregiver, they make the medical decisions when it comes to their children, said Dr. Chennankara.

“For their own longevity, when it comes to endurance, they need to know that this is a marathon and not a sprint,” said Dr. Chennankara.

Second, be honest with yourself, your kids, and your peers. Manage stigmas in the ways you see fit.

“I don’t think there’s a one size fits all approach. So, recognizing and being aware of the stigmas that the parents have, or the child has, or a family member has, and bringing that to the light, and processing that, and working through that is important first,” said Dr. Chennankara.

After working through stigmas with yourself and your children, you can extend these practices with your next circle of friends and family, said Dr. Chennankara.

Third, be an advocate. You never know what people are going through, so make sure you’re intentional with the actions you perform and the thoughts you vocalize.

There are different levels of advocacy, depending on where you feel like you’re at right now. Something you can do is share testimonies from your child as a way to reduce the stigma around mental health.

Having these conversations helps kids know that they aren’t alone in struggling with mental illness. 

“One in four kids go through some type of mental distress throughout their developmental period,” said Dr. Chennankara.

Fourth, remember that your best is good enough. Don’t let your opinions get clouded by too many other people.

“There is no such thing as perfect parents. And, a child does not need a perfect parent. They need a present parent,” said Dr. Chennankara.

Remember that asking for help doesn’t make you weak or a failure, it leads to more awareness. Take advantage of all the resources you can.

In order for kids to trust caregivers with the big things, they need to listen to the little things.

Being a pediatrician, Dr. Bianka Soria-Olmos knows how to identify the signs of autism spectrum disorder (ASD) in her patients. When her own son was diagnosed, she learned the very personal challenges that come along with being a parent of a child with ASD. On this episode of Raising Joy, we talk to Dr. Soria-Olmos about how the diagnosis and how it has impacted her work as a doctor and as a mom.

Raising a child who struggles with their mental health can be scary, challenging and confusing. The ACEs Task Force of Tarrant County developed a video series called The Roadmap for Children’s Mental Health to help families navigate this health issue.

On episode 41 of Raising Joy, Dr. Brian Dixon from Mindful – a psychiatric practice based in Fort Worth and Frances Wampler from the Center for Children’s Health led by Cook Children’s share the mission behind these videos, and how Wampler’s own experience with her child informed the project.

If your child is struggling with their emotions and you don’t know where to turn, this episode is a great start. Mental and behavioral health services can have long waiting lists. If your child is not experiencing a mental health crisis, they may be able to wait to see a professional. Sometimes waiting can be frustrating and take a toll on everyone.

Tips for what you can do while you wait for an appointment with a mental health professional.

First, provide as much structure as possible. Some examples include having a consistent time that your children wake up, having a day-to-day schedule, and making sure they’re going to bed around the same time.

“I think all human beings [rely on structure], adults included, but for kids, it’s even more important that you stay as structured as possible without micromanaging,” said Dr. Brian Dixon, a psychiatrist in Fort Worth.

Physical structure is just as important, such as a decluttered house to come home to.

“Kids are similar to adults in that when we go into chaos, we feel more chaotic and when we feel more chaotic, we tend to make decisions that aren’t always in our best interest,” said Dr. Dixon.

Second, provide reminders that your children are safe and in good hands.

“You know your child better than anybody else,” Dr. Dixon said. “You know if things are going a little bit off the beaten pathway, if they’re not eating as much, if they’re getting more withdrawn or if their personality is changing. All of those are signs that you need to kind of perk your ears up and pay very, very close attention to what your kiddo is doing.”

“If you have that gut instinct that something is going on safety-wise, make sure to reach out,” Dr. Dixon said. Call 911 if it’s an emergency. Talk to your child’s doctor or psychiatrist to get access to care as soon as possible.

Additional ways to comfort your children can include meditation, prayer, and calm music.

Third, spend quality time with your children. Some examples of this can include reading a book or cooking together.

Kids are microphone sponges, Dr. Dixon said.

“They soak in everything that you’re putting out and then they, like a microphone, amplify those same things,” Dr. Dixon said.

Being mindful and thoughtful of the way you carry yourself will have that effect on your children as well.

“If you do those things, they will help prevent a lot of the triggers from happening so that then the kiddo doesn’t have to use the coping skill and then end up in an emergency situation,” said Dr. Dixon.

Fourth, don’t give up.

While it’ll be difficult and there will be moments of frustration for both you and your child, don’t give up.

Remember to take a step back and breathe. Don’t be afraid to reach out for help or ask for support from friends and family.

The waiting period can be exhausting, but Dr. Dixon recommends focusing on diet, sleep, and exercise. This includes getting quality sleep and helping your children get movement throughout the day.

Suzy DeYoung didn’t plan on devoting her life to the subject of trauma, however, an accident involving her child led her down a path of exploring some of the most difficult subjects imaginable. More than 113,000 children in Tarrant County live in poverty.
